What happens

Mr. Burns, the frail, very thin, stooped old man with a hooked nose and grey hair at the sides, is at first seen with a large syringe being jabbed toward him by a gloved hand at the top of the frame, and then he is in a hospital setting. A masked man with spiky black hair and a bulky grey sweater, who may be Carl or Lenny but is not clearly identified, stands beside a dental-style chair while a gaping, toothy Burns with a bib-like white cloth hangs over the chair edge, and the masked man looks wide-eyed and uneasy. The masked man then stares upward and sideways in alarm, with tiled ceiling visible above. Finally Burns, wearing a long pale green robe and barefoot, staggers out with arms spread wide beside a glass emergency door marked EMERGENCY, next to a NO PARKING sign. Visible objects include a wall rack of dental tools, a hanging suction hose, and a blue-lidded syringe.
